CBD is available in a number of different formulations including creams, tablets, oils, and gummies. These can vary in terms of their ingredients as well as dosages, and there is not a great deal of research available on what dose might be beneficial or safe to treat certain conditions.

Cannabidiol (CBD) is the second most abundant cannabinoid found in marijuana. Unlike tetrahydrocannabinol (THC), CBD does not have psychoactive effects. Interest in the use of CBD for health purposes has grown tremendously in the last few years.

CBD is believed to have a range of positive physical and mental health effects. Because of this, it has become increasingly popular as a way to alleviate everything from anxiety to sleep disorders.

In order to determine if CBD is right for you, it is important to consider its potential benefits, side effects, and available research on safe dosages.

Possible Benefits

CBD is just one of hundreds of different compounds found in the cannabis plant. While cannabis has been used in holistic medicine for many years, only recently have researchers begun to explore some of the medicinal purposes for CBD and other cannabinoids.

While further research is still needed, there is some evidence that CBD may have some beneficial mental health effects. These include:

  • Alleviating depression: Some research also indicates that CBD may be useful as a treatment for depression. Studies suggest that the cannabinoid might have an influence on how the brain responds to serotonin, a neurotransmitter that plays a key role in mood.   People with depression sometimes have a low level of serotonin, so CBD may help the brain use available serotonin more effectively.
  • Improving sleep: While the reasons are not entirely understood and require further research, CBD also appears to have potential as a treatment for sleep problems. For example, one study found that people who took CBD also reported improvements in the quality of their sleep.  
  • Reducing anxiety: Anxiety is one of the most common types of mental health conditions, affecting almost 20% of American adults each year. Research suggests that CBD may help alleviate acute symptoms of a number of anxiety-related conditions including generalized anxiety disorder, obsessive-compulsive disorder, and social anxiety disorder.  

In addition to the mental health benefits, CBD may also have therapeutic benefits for a range of other conditions. The World Health Organization suggests that CBD may have beneficial effects in the treatment of:  

  • Alzheimer’s disease
  • Arthritis
  • Cardiovascular disease
  • Diabetes
  • Infection
  • Inflammation
  • Multiple sclerosis
  • Nausea
  • Pain
  • Psychosis

It is important to remember that these benefits have not yet been conclusively proven. More research is needed to determine the role that CBD might play in the treatment of different disorders and health conditions.

Research

There have been a number of studies that suggest that CBD may have a number of different physical and mental health uses. However, more research is still needed to better understand the substance’s potential applications and possible long-term side effects.

A 2019 comprehensive review published in The Lancet Psychiatry looked at previously published studies.   The review ultimately concluded that there was little evidence to support the use of CBD for mental health purposes and suggested that more research is needed in order to substantiate its use to treat symptoms of conditions such as anxiety, depression, and insomnia.

It is important to remember that this doesn’t mean that CBD isn’t effective. Many of the studies that were included in the review were small, had few participants, and were not randomized controlled trials.

This suggests that more research involving more participants and well-designed studies is needed in order to better understand if, how, and why CBD works.

While its effectiveness is still up for debate, one 2017 review found that it was a relatively safe option.   While it is important to remember that there is still a great deal we don’t yet know about CBD and its effects, it is something that you might opt to try to see if you experience any benefits.

How Much Should You Take?

The dosages used in research studies vary and there is no consensus on how much should be used for specific conditions. If you do decide to try CBD, it is also important to note that there is no universally agreed upon dose. Research also suggests that people may respond differently to various dosages, so the amount that is right for your needs might vary.

CBD Dosages

Some dosages that have been used in research studies for different conditions include:

  • Anxiety: 300 to 600 mg  
  • Bowel disease: 10 mg per day  
  • Cancer-related pain: 50 to 600 mg per day  
  • Parkinson’s disease: 75 to 300 mg per day  
  • Poor sleep: 25 mg per day  
  • Psychosis: 600 mg per day  

One 2020 review of studies found that participants showed improvements in anxiety levels after single doses of CBD ranging from 300 to 600 mg.   Such results indicate that the CBD may hold promise as a treatment to alleviate symptoms of acute anxiety.

It is important to remember that you should always talk to your doctor before using CBD if you have symptoms of a serious mental or physical health condition. CBD could potentially worsen symptoms or interact with other medications you are taking.

Looking at the dosage information for the CBD product that has been FDA approved can also be helpful. For Epidiolex, an FDA-approved cannabis-derived medication used to treat seizures in people with certain types of epilepsy, the starting dosage is 5 milligrams per kilogram of body weight. This dose can later be increased to 5 mg per kilogram of body weight twice a day.

Other CBD products are not FDA regulated and do not have officially recommended dosages. This can make it difficult to determine how much you might need, but there are some things you can consider that might help.

  • Assess your sensitivity to CBD: Your individual ability to tolerate CBD can also play a role in determining how much you need. If you are very sensitive to the effects of CBD, you should take a small dose. Some people may find that they are not as affected by the substance, so they may need to take a larger dose to notice any beneficial effects.
  • Consider individual factors: When you are trying to decide how much CBD to take, there are a number of factors you should consider. These include the formulation and concentration of the capsule, oil drops, or gummies you are taking, the symptoms you are treating, and your age, sex, weight, and overall health. Generally, people with heavier bodies need to take a little more to achieve the same effects. Men may need a larger dose, while older people may need less.
  • Consider the symptoms you’re treating: The symptoms you are trying to alleviate can also play a role in the CBD dosage you need to take to see results. In one study, participants who took 25mg of CBD each day had improved sleep quality, although the results were not consistent.   However, you might find that you need a lower or higher dose if you are treating another type of condition.
  • Try a dosage calculator: Researchers note that while the variety of dosing strategies and formulations make it difficult to determine efficacy, there are a number of online “dose-calculators” available online (such as mydosage.com) that are designed to help people choose the correct dose.   The accuracy of such calculators is difficult to assess, but it may be a good place to start.

Before you try CBD, discuss your plan with your doctor. They may be able to recommend a dose and help you better understand any potential risks, complications, side effects, or interactions you might experience.

Start With a Low Dose

Unless your doctor recommends a specific dose, start by taking 10 to 20 mg a day. Take this for a week to ensure that it is well-tolerated and that you don’t experience any unwanted effects or an allergic reaction.

If this dose does not have the desired effect, try increasing in increments of 5mg each week until the desired amount is reached.

In studies, amounts vary from as low as 20 milligrams per day to up to 1,500 milligrams (mg) per day. The World Health Organization reports that dosages in clinical research studies typically range between 100 and 800 milligrams per day.  

Is It Possible to Take Too Much?

So what is the maximum amount of CBD you should take? Researchers have found that 600 mg per day appears to be safe, but one study suggested that doses of up to 1,500 mg a day are safe and tolerated well.  

However, it’s important to remember that research is still in its infancy and experts do not yet fully understand the potential long-term impacts of CBD usage. For that reason, you should always discuss your CBD use with your doctor.

Starting at a lower dose and working your way up to the amount you need may be the best ways to avoid taking too much.

How to Take CBD

The amount of CBD found in a product may depend on different factors, including the formulation and method of administration. CBD products are available in a number of different forms including oils, capsules, tablets, nasal sprays, and gummies.

One of the most popular ways to take CBD is as an oil. Such products are made by combining CBD with some type of carrier oil, such as coconut oil. Some more recently developed products include dietary supplements, foods, beverages, lotions, salves, and cosmetics.

The type of CBD product you choose may depend on what you are trying to treat. If you are looking for general mood improvements, a dietary supplement might be a good option.

If you are targeting specific symptoms of a condition, taking an oil, capsule, or gummy might be a better way to obtain a higher, more concentrated dose.

Topical applications may produce localized effects, but they are unlike to have any mental health benefits.

What Kind Should You Take?

It’s also important to remember that many products don’t contain just CBD on its own. There are three types of CBD available:

  • Isolate contains CBD and only CBD.
  • Broad-spectrum contains CBD and other cannabinoids, but not THC.
  • Full-spectrum contains CBD, THC, and other cannabinoids.

It may be helpful to take a broad-spectrum product since research suggests that CBD’s effects may be most beneficial when taken in conjunction with other cannabinoids, a phenomenon known as the entourage effect. CBD may also help mitigate some of the effects of THC.

Side Effects

While CBD is generally well-tolerated, this does not mean that you won’t experience any side effects.

Some of the most common side effects that people experience when taking CBD include:

  • Appetite changes
  • Diarrhea
  • Nausea
  • Dizziness
  • Stomach upset
  • Weight changes

Some recent research has generated concerns over the safety and potential long term effects of CBD. One study involved giving mice an equivalent of the maximum dose of the CBD medication Epidiolex, which is used to treat certain forms of epilepsy. The results indicated an increased risk for liver damage as well as concerns over its interaction with other medications.  

Safety

It is also important to remember that CBD products are not regulated by the U.S. Food and Drug Administration (FDA). Some manufacturers make unproven claims about the uses and efficacy of their products. There is also concern about the quality and safety of the products themselves.

One report by the Centers for Disease Control and Prevention (CDC) indicated that a number of people experienced negative unwanted side effects due to CBD products that contained synthetic CBD, although the products were not labeled as containing such ingredients.  

Mislabeling appears to be a fairly common problem with CBD products. In one study, 70% of the CBD products that were sold online contained significantly more of the psychoactive ingredient THC than the label indicated.  

Federal law prohibits the sale of products that contain more than 0.3% THC. States laws also vary, so you should always check with your state before buying CBD products online.

Health Benefits of CBD for RA

Animal studies have suggested that CBD has pain-relieving and anti-inflammatory properties, but these effects have not been validated with quality studies in humans. Anecdotally, some people who have tried CBD for treatment of arthritis symptoms report noticeable pain relief, improved sleep improvement, and reduced anxiety.

The trials that have been conducted in humans have not yielded strong evidence for the use of CBD in managing arthritic pain.

  • A randomized trial of topical CBD for treatment of knee osteoarthritis lasted only 12 weeks, and results were mixed.
  • One of the largest reviews examining the health effects of cannabis and CBD concluded that there is substantial evidence that cannabis is an effective treatment for chronic pain in adults. There was, however, no specific conclusion regarding CBD, as definitive studies were not available.
  • More recently, a new study showed that CBD can potentially help alleviate RA pain and inflammation by acting on immune cells that are attacking healthy cells in the body. It’s important to note that this study was also not conducted with human subjects.

Research in this area is ongoing.

Given the lack of strong scientific evidence supporting the use of CBD, it is not recommended as the first choice for pain relief in RA.

If you have RA, you should not stop taking your prescribed medications that may be protecting your joints from future damage. You should discuss any changes you want to make to your medication regimen with your doctor.

Additional Health Benefits

The strongest scientific evidence for effectiveness of CBD as a medical treatment is for seizure prevention in Dravet syndrome and Lennox-Gastaut syndrome, which are severe childhood epilepsy disorders that typically don’t respond to antiseizure medications. Several years ago, the FDA approved the first cannabis-derived medicine for these conditions, Epidiolex, which contains CBD.

Other potential health benefits of CBD include:

  • Reduce anxiety, insomnia, and depression
  • Relieve cancer or cancer treatment symptoms
  • Clear acne
  • Slow the progress of neurologic disease like Alzheimer’s disease

Possible Side Effects

Treatment with CBD should be started slowly and under the supervision of a doctor, who will oversee its effectiveness and side effects. There have been reports of mild side effects of CBD.

The most common side effects include:

  • Nausea
  • Fatigue
  • Irritability
  • Diarrhea
  • Dry mouth
  • Lightheadedness
  • Sleepiness
  • Low blood pressure
  • Appetite changes

It’s important to note that CBD may interact with other drugs, including some of the medications that are prescribed for the treatment of RA.

Warnings and Interactions

There are some warnings and adverse drug interactions to be aware of before beginning using CBD for management of RA-associated pain.

Warnings

Do not stop taking any other treatments without consulting your doctor. Additionally, CBD shouldn’t be a replacement for other therapies you are using.

Pregnant people and children should consult a doctor before using CBD since there isn’t enough research on the effects of CBD on these populations. There has been concern about a possible link between inhaled cannabis and lower-birthweight babies, but it’s not clear if this applies to CBD.

Currently, the U.S. Food and Drug Administration (FDA) does not regulate the safety and purity of CBD products, so you cannot know for sure that the product you buy has active ingredients at the dose listed on the label. The product may contain other unknown elements. The most effective therapeutic dose of CBD for any particular medical condition has not been established.

Are CBD Products Legal?

CBD products derived from hemp are no longer considered schedule I drugs under the federal Controlled Substances Act, but they still remain in a legal gray zone.   There are changes underway on federal and state levels that will ultimately clarify the laws and regulations related to CBD-based products and sales. Despite that, they’re widely available in nearly every state and online. People who want to use CBD should check their own state laws.

Interactions

Studies have found moderate interactions between CBD and medications commonly prescribed for treating RA.

Interactions can occur with these medications:

  • Corticosteroids
  • Tofacitinib
  • Naproxen
  • Celecoxib
  • Tramadol
  • Some antidepressants, including amitriptyline, citalopram, fluoxetine, mirtazapine, paroxetine, and sertraline
  • Some medications used for treating fibromyalgia, including gabapentin and pregabalin

CBD can increase your level of coumadin, a blood thinner.

CBD may also raise levels of other medications in your blood by the same mechanism that grapefruit juice does.

Medications that interact with grapefruit juice can potentially interact with CBD, including:

  • Cholesterol medications
  • High blood pressure medications
  • Organ-rejection medications
  • Anti-anxiety medications
  • Corticosteroids
  • Heart medications
  • Some antihistamines

Preparation and Dosage

If you’ve decided to try CBD for treating your RA pain, the next step is figuring out how much to take.

Your dosage depends on a number of factors:

  • Body weight
  • Medical indication
  • The concentration and form of CBD that you’re taking

You will likely need to start with a low dose and find what works for you. Studies have found extreme differences in dosages, with some people using 5 milligrams (mg) and others as much as 600 mg.

Your healthcare provider may direct you to start with 20-40 mg per day and increase slowly each day until you feel the relief you’re looking for.

  • When preparing to take a liquid form, be aware that the CBD extract is mixed with a carrier oil, so there are two measures to know: the amount of the liquid product to take (the dose) and the amount of CBD in each dose

If CBD alone doesn’t work and you are in a state where medical or recreational marijuana is legal, you could consider talking to your doctor about taking CBD with a very low-dose THC product. Be aware that even at low levels, THC may make you feel high, resulting in cognitive, motor, and balance impairment.

If you experience any unwanted side effects when using a CBD product, immediately discontinue use and inform your doctor.

Look for the following when choosing a CBD product:  

  • Look for products manufactured in the United States with ingredients grown domestically

What did the researchers investigate?

The researchers evaluated whether cannabis or cannabis oil (cannabidiol) was better than placebo (e.g. fake drug) for treating adults with active ulcerative colitis or ulcerative colitis that is in remission. The researchers searched the medical literature extensively up to 2 January 2018.

What did the researchers find?

Two studies including 92 adult participants with ulcerative colitis were included. Both studies assessed cannabis therapy in participants who had active ulcerative colitis. No studies that assessed cannabis therapy in participants with ulcerative colitis in remission were identified. One study (60 participants) compared 10 weeks of treatment with capsules containing cannabis oil with up to 4.7% D9-tetrahydrocannabinol (THC) to placebo in participants with mild to moderately active ulcerative colitis. The starting dose of cannabidiol was 50 mg twice daily which was increased, if tolerated, to a target of 250 mg twice daily. The other study (32 participants) compared 8 weeks of treatment with two cannabis cigarettes per day containing 0.5 g of cannabis, corresponding to 11.5 mg THC to placebo cigarettes in participants with ulcerative colitis who did not respond to conventional medical treatment.

The study comparing cannabis oil capsules to placebo found no difference in remission rates at 10 weeks. Twenty four (7/29) percent of cannabidiol participants achieved clinical remission compared to 26% (8/31) of placebo participants. The study also showed higher self reported quality of life scores in cannabis oil participants compared to placebo participants. More side-effects were observed in the cannabis oil participants compared to the placebo participants. These side effects were considered to be mild or moderate in severity. Common reported side effects include dizziness, disturbance in attention, headache, nausea and fatigue. No patients in the cannabis oil group had any serious side effects. Ten per cent (3/31) of the placebo group had a serious side effect. Serious side effects in the placebo group included worsening ulcerative colitis and one complicated pregnancy.

The second study comparing two cannabis cigarettes (23 mg THC/day) to placebo cigarettes showed lower disease activity index scores in the cannabis group compared to the placebo group. C-reactive protein and fecal calprotectin levels (both measures of inflammation in the body) were similar in both groups. No serious side effects were reported. This study did not report on remission rates.

Conclusions

The effects of cannabis and cannabis oil on ulcerative colitis are uncertain, thus no firm conclusions regarding the effectiveness and safety of cannabis or cannabis oil in adults with active ulcerative colitis can be drawn. There is no evidence for cannabis or cannabis oil use for maintenance of remission in ulcerative colitis. Further studies with a larger number of participants are required to assess the effects of cannabis in people with active and inactive ulcerative colitis. Further, cannabis has potential for adverse events and the long-term consequences of cannabis and cannabinoid use in UC are unknown.

To assess the efficacy and safety of cannabis and cannabinoids for the treatment of patients with UC.

We searched MEDLINE, Embase, WHO ICTRP, AMED, PsychINFO, the Cochrane IBD Group Specialized Register, CENTRAL, ClinicalTrials.Gov and the European Clinical Trials Register from inception to 2 January 2018. Conference abstracts and references were searched to identify additional studies.

Randomized controlled trials (RCTs) comparing any form or dose of cannabis or its cannabinoid derivatives (natural or synthetic) to placebo or an active therapy for adults (> 18 years) with UC were included.

Two authors independently screened search results, extracted data and assessed bias using the Cochrane risk of bias tool. The primary outcomes were clinical remission and relapse (as defined by the primary studies). Secondary outcomes included clinical response, endoscopic remission, endoscopic response, histological response, quality of life, C-reactive protein (CRP) and fecal calprotectin measurements, symptom improvement, adverse events, serious adverse events, withdrawal due to adverse events, psychotropic adverse events, and cannabis dependence and withdrawal effects. We calculated the risk ratio (RR) and corresponding 95% confidence interval for dichotomous outcomes. For continuous outcomes, we calculated the mean difference (MD) and corresponding 95% CI. Data were pooled for analysis when the interventions, patient groups and outcomes were sufficiently similar (determined by consensus). Data were analyzed on an intention-to-treat basis. GRADE was used to evaluate the overall certainty of evidence.

Two RCTs (92 participants) met the inclusion criteria. One study (N = 60) compared 10 weeks of cannabidiol capsules with up to 4.7% D9-tetrahydrocannabinol (THC) with placebo capsules in participants with mild to moderate UC. The starting dose of cannabidiol was 50 mg twice daily increasing to 250 mg twice daily if tolerated. Another study (N = 32) compared 8 weeks of therapy with two cannabis cigarettes per day containing 0.5 g of cannabis, corresponding to 23 mg THC/day to placebo cigarettes in participants with UC who did not respond to conventional medical treatment. No studies were identified that assessed cannabis therapy in quiescent UC. The first study was rated as low risk of bias and the second study (published as an abstract) was rated as high risk of bias for blinding of participants and personnel. The studies were not pooled due to differences in the interventional drug.

The effect of cannabidiol capsules (100 mg to 500 mg daily) compared to placebo on clinical remission and response is uncertain. Clinical remission at 10 weeks was achieved by 24% (7/29) of the cannabidiol group compared to 26% (8/31) in the placebo group (RR 0.94, 95% CI 0.39 to 2.25; low certainty evidence). Clinical response at 10 weeks was achieved in 31% (9/29) of cannabidiol participants compared to 22% (7/31) of placebo patients (RR 1.37, 95% CI 0.59 to 3.21; low certainty evidence). Serum CRP levels were similar in both groups after 10 weeks of therapy. The mean CRP in the cannabidiol group was 9.428 mg/L compared to 7.638 mg/L in the placebo group (MD 1.79, 95% CI -5.67 to 9.25; moderate certainty evidence). There may be a clinically meaningful improvement in quality of life at 10 weeks, measured with the IBDQ scale (MD 17.4, 95% CI -3.45 to 38.25; moderate certainty evidence). Adverse events were more frequent in cannabidiol participants compared to placebo. One hundred per cent (29/29) of cannabidiol participants had an adverse event, compared to 77% (24/31) of placebo participants (RR 1.28, 95% CI 1.05 to1.56; moderate certainty evidence). However, these adverse events were considered to be mild or moderate in severity. Common adverse events included dizziness, disturbance in attention, headache, nausea and fatigue. None (0/29) of the cannabidiol participants had a serious adverse event compared to 10% (3/31) of placebo participants (RR 0.15, 95% CI 0.01 to 2.83; low certainty evidence). Serious adverse events in the placebo group included worsening of UC and one complicated pregnancy. These serious adverse events were thought to be unrelated to the study drug. More participants in the cannabidiol group withdrew due to an adverse event than placebo participants. Thirty-four per cent (10/29) of cannabidiol participants withdrew due to an adverse event compared to 16% (5/31) of placebo participants (RR 2.14, 95% CI 0.83 to 5.51; low certainty evidence). Withdrawls in the cannabidiol group were mostly due to dizziness. Withdrawals in the placebo group were due to worsening UC.

The effect of cannabis cigarettes (23 mg THC/day) compared to placebo on mean disease activity, CRP levels and mean fecal calprotectin levels is uncertain. After 8 weeks, the mean disease activity index score in cannabis participants was 4 compared with 8 in placebo participants (MD -4.00, 95% CI -5.98 to -2.02). After 8 weeks, the mean change in CRP levels was similar in both groups (MD -0.30, 95% CI -1.35 to 0.75; low certainty evidence). The mean fecal calprotectin level in cannabis participants was 115 mg/dl compared to 229 mg/dl in placebo participants (MD -114.00, 95% CI -246.01 to 18.01). No serious adverse events were observed. This study did not report on clinical remission, clinical response, quality of life, adverse events or withdrawal due to adverse events.

What is it?

Cannabidiol is a chemical in the Cannabis sativa plant, also known as marijuana or hemp. Over 80 chemicals, known as cannabinoids, have been identified in the Cannabis sativa plant. While delta-9-tetrahydrocannabinol (THC) is the major active ingredient in marijuana, cannabidiol is also obtained from hemp, which contains only very small amounts of THC.

The passage of the 2018 Farm Bill made it legal to sell hemp and hemp products in the U.S. But that doesn’t mean that all hemp-derived cannabidiol products are legal. Since cannabidiol has been studied as a new drug, it can’t be legally included in foods or dietary supplements. Also, cannabidiol can’t be included in products marketed with therapeutic claims. Cannabidiol can only be included in “cosmetic” products and only if it contains less than 0.3% THC. But there are still products labeled as dietary supplements on the market that contain cannabidiol. The amount of cannabidiol contained in these products is not always reported accurately on the product label.

Cannabidiol is most commonly used for seizure disorder (epilepsy). It is also used for anxiety, pain, a muscle disorder called dystonia, Parkinson disease, Crohn disease, and many other conditions, but there is no good scientific evidence to support these uses.

How effective is it?

A specific cannabidiol product (Epidiolex, GW Pharmaceuticals) has been shown to reduce seizures in adults and children with various conditions that are linked with seizures. This product is a prescription drug for treating seizures caused by Dravet syndrome, Lennox-Gastaut syndrome, or tuberous sclerosis complex. It has also been shown to reduce seizures in people with Sturge-Weber syndrome, febrile infection-related epilepsy syndrome (FIRES), and specific genetic disorders that cause epileptic encephalopathy.   • Diabetes. Early research shows that taking cannabidiol does not improve blood glucose control adults with type 2 diabetes.
  • A movement disorder marked by involuntary muscle contractions (dystonia). It is unclear if cannabidiol is beneficial for dystonia.
  • An inherited condition marked by learning disabilities (fragile- X syndrome). Early research suggests that applying cannabidiol gel might reduce anxiety and improve behavior in children with fragile X syndrome.
  • A condition in which a transplant attacks the body (graft-versus-host disease or GVHD). Graft-versus-host disease is a complication that can occur after a bone marrow transplant. Early research has found that taking cannabidiol daily starting 7 days before bone marrow transplant and continuing for 30 days after transplant can extend the time it takes for a person to develop GVHD.
  • An inherited brain disorder that affects movements, emotions, and thinking (Huntington disease). Early research shows that taking cannabidiol daily does not improve symptoms of Huntington disease.
  • Multiple sclerosis (MS). Early research suggests that using a cannabidiol spray under the tongue might improve pain and muscle tightness in people with MS.
  • Withdrawal from heroin, morphine, and other opioid drugs. Early research shows that taking cannabidiol for 3 days might reduce cravings and anxiety in people with heroin use disorder.
  • Parkinson disease. Early research shows that cannabidiol might reduce anxiety and psychotic symptoms in people with Parkinson disease.
  • Schizophrenia. Early research suggests that taking cannabidiol improves symptoms and wellbeing in people with schizophrenia.
  • Quitting smoking. Early research suggests that inhaling cannabidiol with an inhaler for one week might reduce the number of cigarettes smoked by smokers trying to quit.

  • A group of painful conditions that affect the jaw joint and muscle (temporomandibular disorders or TMD). Early research shows that applying an oil containing cannabidiol to the skin might reduce pain in people with TMD.
  • Nerve damage in the hands and feet (peripheral neuropathy).
  • Bipolar disorder.
  • Insomnia.
  • Other conditions.

More evidence is needed to rate the effectiveness of cannabidiol for these uses.

How does it work?

Cannabidiol has effects on the brain. The exact cause for these effects is not clear. However, cannabidiol seems to prevent the breakdown of a chemical in the brain that affects pain, mood, and mental function. Preventing the breakdown of this chemical and increasing its levels in the blood seems to reduce psychotic symptoms associated with conditions such as schizophrenia. Cannabidiol might also block some of the psychoactive effects of delta-9-tetrahydrocannabinol (THC). Also, cannabidiol seems to reduce pain and anxiety.

Are there safety concerns?

When taken by mouth: Cannabidiol is POSSIBLY SAFE when taken by mouth or sprayed under the tongue appropriately. Cannabidiol in doses of up to 300 mg daily have been taken by mouth safely for up to 6 months. Higher doses of 1200-1500 mg daily have been taken by mouth safely for up to 4 weeks. A prescription cannabidiol product (Epidiolex) is approved to be taken by mouth in doses of up to 25 mg/kg daily. Cannabidiol sprays that are applied under the tongue have been used in doses of 2.5 mg for up to 2 weeks.

Some reported side effects of cannabidiol include dry mouth, low blood pressure, light headedness, and drowsiness. Signs of liver injury have also been reported in some patients, but this is less common.

When applied to the skin: There isn’t enough reliable information to know if cannabidiol is safe or what the side effects might be.

Special precautions & warnings:

Pregnancy and breast-feeding: Cannabidiol is POSSIBLY UNSAFE to use if you are pregnant or breast feeding. Cannabidiol products can be contaminated with other ingredients that may be harmful to the fetus or infant. Stay on the safe side and avoid use.

Children: A prescription cannabidiol product (Epidiolex) is POSSIBLY SAFE when taken by mouth in doses up to 25 mg/kg daily. This product is approved for use in certain children 1 year of age and older.

This has spurred researchers to investigate whether there are drug–drug interactions (DDIs) and/or adverse drug events (ADEs) between these traditionally used anti-epileptic drugs and CBD. Data showed that, although there were no serious ADEs, deaths, or pregnancies during the trial, most subjects reported some ADEs of mild severity, while 10.4% subjects reported moderate ADEs, and 2.6% subjects reported severe ADEs, with severe events being characterized by popular rashes. Other moderate ADEs included feeling intoxicated, menstrual discomfort or other mild rashes.

Despite these ADEs, the researchers concluded that the effective dose of 750 mg of cannabidiol (approximately 20 mg/kg/day for a 75‐kg adult), was mostly well tolerated when co-administered with clobazam, stiripentol, or valproate and that ADEs only occurred in patients for which there was no dosage titration.

In another study investigating the interactions between CBD and commonly used anti-epileptic drugs, concluded that, although serum levels of the drugs topiramate, rufinamide, and N‐desmethylclobazam were found when use in conjunction with and increased CBD dosages. However, all changes were within the accepted therapeutic range but did underscore the importance of monitoring serum AED levels and LFTs during treatment with CBD.

In yet another such study, but with researchers this time looking into the clinical implications and importance of DDIs between anticancer agents and CBD in patients with cancer, the reviewers found that there was limited information available, with most of the data coming from in vitro studies and that the true in vivo implications are not well-known. This lead them to believe that erring on the side of caution is the best option, and that doctors and pharmacists should always consider the possibility of interactions and their consequences whenever they are aware of a patient using CBD products.

Similarly, a study investigating the potential ADEs and DDIs the researchers highlighted that medical CBD users under clinical supervision should be screened for potential DDIs and ADEs between CBD, other pharmacotherapies, and their underlying conditions. They also recommended that an increase in awareness is needed among the lay public who are recreational or consumer CBD users. Similarly, healthcare providers should also be made aware of the potential for DDIs and ADEs with CBD and strategically prescribe and manage patient regimens while also considering patient desires for complementary or alternative therapies.

In other words, the inactive compound is ingested, and once in the body, it is processed into its active compound.

Hypertension is the medical term for high blood pressure. As blood is pumped through your blood vessels, they undergo a degree of force. If you have high blood pressure, this force can damage the walls of your blood vessels, veins and arteries.

When you visit the doctor, you’ll usually hear your blood pressure described by two numbers, like 120/70.

The first number refers to the systolic pressure, the pressure exerted when the heart contracts and pumps blood into the arteries. The second is the diastolic pressure, measured when the heart muscle relaxes between beats. In cardiology, a systolic pressure of 140, and/or a diastolic pressure of 90, is considered the boundary between normal and high blood pressure.

High blood pressure can lead to cardiovascular complications such as heart disease, heart attack, stroke, aneurysm and heart failure. The condition is exacerbated by type 2 diabetes, stress, anxiety, inflammation, high cholesterol levels, insomnia, a diet high in salt, and alcohol and tobacco use.

Lowering High Blood Pressure: CBD and Blood Pressure

High blood pressure can be naturally lowered by reducing stress, salt intake, and consumption of alcohol and tobacco. You can also lower your blood pressure by exercising regularly and eating more fruits and vegetables.

Your doctor may prescribe medications that can lower blood pressure. Many of these medications, such as calcium channel blockers and ACE-inhibitors, cause side effects like headaches, dizziness, constipation and swollen ankles. That has naturally sparked interest in non-prescription treatments for high blood pressure.

New evidence links CBD to a decrease in blood pressure. For many people, CBD oil has fewer side effects than prescription medications, too.

We spoke with Rebecca Park, registered nurse and founder of RemediesForMe.com, who told us that CBD oil tinctures made using hemp have very few side effects and are generally well tolerated.

If you are already on high blood pressure medications, however, consult your physician before using CBD oil as there may be interactions.

What is CBD Oil?

Cannabidiol, better known as CBD, is a non-psychoactive phytocannabinoid, which is a cannabinoid found in the two forms of cannabis, marijuana and hemp plants.

Commercially-available CBD oil is cannabidiol extracted from the hemp plant, often mixed with coconut or hemp oil. Unlike its marijuana-derived counterpart, this CBD oil does not contain high levels of tetrahydrocannabinol (THC), so cannabidiol does not get you high. Instead, most users report that it makes them feel relaxed and calm, among the other generally positive effects of CBD.

According to federal law, hemp-derived CBD oil is classified as having less than 0.3% THC, making it legal to purchase and use. And since hemp has minuscule amounts of THC content, CBD oil from this type of cannabis plant does not cause any of THC’s intoxicating side effects. Marijuana-derived CBD oil, whether it’s derived from sativa or indica-dominant plants, contains more than 0.3% THC as its primary active ingredient, and may make you high. It is only legal in states with recreational or medical marijuana legalization.

Though it comes in a wide variety of forms such as capsules, vape juice, powders, topical patches and lotions, and edibles like gummies, cannabidiol is most popularly consumed as a sublingual oil. You simply place a few drops of CBD oil under your tongue and let it sit for 30 to 90 seconds.

Pharmacology research has shown a wide range of potential benefits for cannabidiol. CBD oil is commonly used to treat or supplement treatment of medical conditions like chronic pain, inflammation, arthritis, multiple sclerosis, Parkinson’s disease and epilepsy. Park added that CBD oil is also commonly used by people suffering from a wide range of mental disorders.

“CBD can be effective in treating generalized anxiety disorder (GAD), social anxiety disorder, anxiety related insomnia, and post-traumatic stress disorder (PTSD),” she said.

Additionally, emerging evidence shows that CBD may use the same mechanism to treat high blood pressure as it does to reduce anxiety.

How CBD Interacts with the Endocannabinoid System and Body

The endocannabinoid system is a biological system in your body that allows cannabinoids to send and receive messages that control most of the body’s important functions. In fact, your body naturally produces cannabinoids every day.

This system has two main endocannabinoid receptors, called CB1 and CB2. THC binds directly to the CB1 receptors, affecting areas of the brain that influence memory, motor coordination and time perception, and to the CB2 receptors which affect the pain centers in your brain and manage inflammation in the body. The interaction between THC and the endocannabinoid system is what causes the psychoactive side effects that marijuana is best known for.

On the other hand, cannabidiol does not directly bind to the CB1 and CB2 receptors. Instead, it causes a number of indirect actions leading to therapeutic benefits, such as activating TRPV1 receptors that regulate pain, body temperature, and inflammation.

Does CBD Oil Lower Blood Pressure?

While Park told us that high blood pressure can be caused by a number of factors, CBD oil appears to lower increases in blood pressure caused by stress and anxiety.

She explained that when emotional or physical stress is high, our bodies release a hormone called cortisol. High levels of cortisol can raise blood pressure.

“CBD lowers blood pressure by reducing cortisol levels and dilating blood vessels, allowing more blood flow and leading to lower blood pressure,” Park said.

Park pointed to a study of 11 participants that showed CBD oil may lower the levels of cortisol produced in the body. This study showed that volunteers saw a significant reduction in cortisol production when given 300 mg and 600 mg of CBD compared to the placebo control.

In a famous 2017 randomized crossover study led by British doctor Khalid Jadoon and published in the Journal of Clinical Investigation (JCI), a single dose of 600 mg of CBD oil or a placebo was administered to nine healthy volunteers. The resulting data showed that acute administration of CBD reduced resting blood pressure compared to placebo dosing.

The patients were then given a series of stress tests to increase heart rate. Those who were given 600 mg of CBD had lower stress-induced blood pressure spikes than the control group.

It’s amazing, but it’s true. A single dose of cannabidiol lowered the blood pressure of patients, whether they were resting or under stress.

The sample size of these studies was small and further research is required to establish whether CBD has a role in the treatment of cardiovascular disorders, but the results are promising.

How CBD Can Help With Hypertension

As Park explained, high blood pressure can be caused by many factors. Fortunately, there are several health benefits of CBD that could indirectly help with hypertension.

Anti-Anxiety

Anxiety can lead to rapid heart rate and palpitations, leaving you at risk for high blood pressure and heart disease.

CBD oil has been shown to reduce anxiety in animal studies by the National Institute for Drug Abuse (NIDA). This study demonstrated that symptoms that accompany anxiety, like high blood pressure and rapid heart rate, were improved by use of CBD.

Reduces Inflammation

Inflammation doesn’t only occur in your joints. In fact, research shows that CBD oil can reduce inflammation in the heart and blood vessels in the brain.

Called a “vasorelaxant” by researchers, CBD oil’s anti-inflammatory properties may lower blood pressure by allowing the free flow of blood through your heart, arteries and veins.

Analgesic Properties

According to the National Institutes of Health (NIH), acute pain can lead to high blood pressure. Because CBD acts on the pain receptors in the brain, the pain-relieving properties of CBD could lead to decreased blood pressure.

CBD Improves Sleep

As CBD reduces stress, you may see an increase in your quality of sleep and general wellness.

Sarah Polansky, owner and developer of the CBD brand Prismatic Plants, told Public Goods that “The more you take CBD and help to regulate your adrenals, the less likely you’ll feel fatigued as your body returns to a state of balance rather than constant stress.”

She added, “This means your need for caffeine consumption could decrease and your quality of sleep could improve, which has a waterfall benefit for increasing overall health.”

Choosing CBD Oil for High Blood Pressure

No prescription or documentation is needed for CBD oil. The only exceptions are the marijuana-derived CBD oil sold in dispensaries (in medical marijuana states, you’ll need a patient card to buy it) and a medication called Epidiolex, a CBD medication only available by prescription to patients suffering from some forms of childhood epilepsy.

All of the forms of CBD that you can buy over the counter are not generic products. Their effectiveness depends largely on the quality of the manufacturer producing it, and the type of CBD that’s used.

Many of the cannabidiol products you’ll see in CBD stores or online are of lower quality, since – as with any product – it’s more expensive to produce high-end merchandise and it sells for a higher price, often with lower profit margins for the vendors.

High-quality CBD oil, capsules, topicals, juice for vaping, or edibles will always produce a better result, so it’s best to do a little research to find reputable vendors who create effective products. Look for oil sourced from Colorado or West Coast organic hemp, extracted with high-end methods (like supercritical CO2 extraction), and containing only a small number of natural or organic ingredients required for flavoring or distributing the cannabidiol (the latter is done via MCT coconut carrier oil in high-quality CBD).

Some cannabidiol delivery methods work more slowly than others.

Capsules and edibles, for example, must make their way through the digestion process before the CBD can be released into the blood stream and carried throughout the body. Vaping or administration under the tongue make the cannabidiol available almost immediately, since it’s absorbed quickly by the sublingual glands or tissues in the lungs. And topical CBD usually doesn’t even make it into the bloodstream, since it’s blocked by the epidermis.

When shopping for CBD oil, you’ll find there are three different types to choose from.

  • Full-spectrum CBD is the most effective. It contains all of hemp’s natural terpenes and flavonoids, as well as all of the cannabinoids in the plant – including the small amount of THC we’ve mentioned. They all work together in the so-called “entourage effect” that boosts CBD’s performance.
  • Broad-spectrum CBD provides a slightly lower entourage effect, because the THC content has been removed from the cannabidiol before it’s processed and sold.
  • CBD isolate goes a step further. All of the components of hemp except cannabidiol are removed, leaving only pure CBD in the final products.

Some people believe that pure cannabidiol does the most good, but in reality, there’s only one reason to avoid CBD that contains THC: it eliminates any chance of turning up dirty on a drug test for marijuana. In reality, very few full-spectrum CBD users ever test positive, but some people’s minds are set at ease by knowing there’s no THC in their body.

What’s The Best CBD Dosage for High Blood Pressure?

Because the FDA has not approved over-the-counter CBD products, there is no official recommendation for a dose of CBD. The right dose depends on your weight, genetics, sex and medical history. More studies need to be conducted with human subjects to get an accurate dosage.

Many CBD oils come in dropperful (1 ml) doses of 10 mg/ml. Cannabidiol affects different people, particularly based on the weight, metabolism and body type. However, Park said the effective therapeutic dose is usually in the range of 20 mg to 300 mg, or 2 to 30 dropperfuls.

In a 2018 study, researchers found that a dose of 300 mg was most effective for reducing anxiety in a simulated public speaking trial. Participants who took 150 mg or 600 mg did not receive the same benefit.

Meanwhile, in the previously-mentioned study of nine healthy volunteers, blood pressure was effectively lowered with a single dose of 600 mg.

The best way to determine how effective a dose of CBD will be for you is to start small and frequently check your blood pressure using an electronic cuff. Write down the dose you took and the time you took it, as well as your blood pressure reading. Over time, you should be able to obtain a clearer picture of what dose will help you achieve low blood pressure.

Park noted that, “If CBD works for you, without the use of other medications, I would say it is a great and safe alternative.” CBD may not be a replacement for exercise and a healthy diet, but it is nice to know you might be able to lower your blood pressure with a few drops of oil under the tongue.
